  • Your values and beliefs: Are there any values or beliefs that are important to you? These could include religious, spiritual, or cultural beliefs.

  • Your loved ones: Who are the important people in your life? Who do you want to be involved in your care decisions?

  • Your wishes for after death: What are your wishes for your funeral or memorial service? Do you have any specific requests regarding your burial or cremation?

It's important to communicate your wishes clearly and openly with your healthcare team. This can help ensure that you receive the care you want and need at the end of life.
